Initial Consultation

Your first visit to ENT Associates establishes a vital foundation for our relationship with you. At your first consult, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.

To make sure there are no delays in care during your first visit experience, please arrive 15 minutes prior to your scheduled appointment to ensure your registration is complete before meeting with your new provider.

Please assist us by being prepared to provide the following:

  • Your insurance card(s)
  • Valid photo ID
  • Office co-pay
  • Your referral authorization and any X-rays, CT scans, MRI films, hearing tests or sleep studies (if applicable). Please bring the actual films on a disk, if possible, along with any printed reports.

It’s recommended that a family member be present to take notes and help with questions especially if the patient is elderly, or there may be issues with hearing loss or a speech barrier.

IMPORTANT: A parent or legal guardian of a minor (under the age of 18) being treated by ENT Associates MUST be present at the initial visit with the physician. Subsequently, to designate another person to consent for your child’s medical care please complete and return this form – click here. The person named must be 18 years of age or older.

For patients who are delayed and arrive late for appointments, every effort will be made to see them the same day. However, wait times may apply, or appointments may need to be rescheduled.

X-Ray/CT/MRI

If your physician has ordered a previous diagnostic study, you may request to have the films and results forwarded to our office. If there is not enough time, please pick them up and bring them to your initial consultation.
